 Until recently, the perception of medical errors among health care providers and the public has been shaped by these anecdotes, and remedies have focused on fixing blame on individual providers, including health plans, hospitals, doctors, pharmacists, nurses, and other caregivers. In addition, hhs continues to expand its efforts to improve patient safety by gathering and analyzing data, conducting relevant research, and educating consumers, businesses and health care providers about preventing medical errors.
